- Conduct thorough patient assessments, including medical history, physical examinations and wellness exams, and diagnostic testing.
- Develop and implement individualized treatment plans based on clinical guidelines and best practices.
- Provide preventive care services, including vaccinations, health screenings, and counseling on lifestyle modifications.
- Diagnose and manage acute and chronic medical conditions, coordinating care with specialists as needed.
- Provide minimal, intermittent call coverage for emergency department to back-up advanced practice providers as needed, including being available for consultation and support during off-hours to ensure continuity of care.
- Provide care and consultation, on a rotational basis, for patients in the Inpatient and Skilled Unit.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Participate in quality improvement initiatives and contribute to the development of clinic policies and procedures.
