StreamMD is a digital health technology company looking for top quality Internal Medicine or Family Medicine Physician Assistants or Nurse Practitioners to use cutting-edge evidence-based technology in providing best-in-class digital primary care. Our goal is to demonstrably improve the treatment of chronic conditions such as hypertension, diabetes, and weight loss using proven state-of-the-art methods and technology, as well as delivering excellent care to patients in New York. We are looking for motivated individuals who want to practice in a unique medical environment. Work with a group that is dedicated to improving both the patient and provider experience.
This is a flexible, part-time opportunity where you can care for patients both in person and via telemedicine. The company will provide all ancillary support staff. We work with a schedule that works for your needs and can range from just 5 hours per week to full-time. Hours available 7 days a week 9am to 9pm for televisits and in-person in our location in Rhinebeck, NY.
Clinicians work independently and are responsible for diagnostic and therapeutic decision-making within the industry and locally established standards of care, clinical practice guidelines, and organization policies and procedures. Qualifications
- Board Certified or Eligible in Family Medicine or Internal Medicine
- Appropriate credentialing for NY state
- Motivated to work independently
- Valid New York medical license
- Active DEA registration
- A commitment to providing best-in-class healthcare with a caring and empathetic demeanor focused on the patient’s emotional and physical well-being.
- Medicare participating physician or willing to become a Medicare participating physician
- Prior enrollment with leading NY health plans is a plus
