What are the responsibilities and job description for the LPN, Assisted Living position at Streamside Assisted Living and Memory Care?
Ohana Ventures is a premier senior living company that is seeking an experienced LPN to join our clinical department. This individual will be responsible for establishing and maintaining a high quality of care service with our Health Services Director for our local Nampa, Idaho community, Streamside Assisted Living and Memory Care. This role would follow policies and procedures; promoting the health and well being of each resident; and assist with directing the training program for clinical staff. As well as auditing and reviewing the communities clinical services regularly to ensure they are meeting and exceeding Ohana standards, assistance in the training and mentoring of unlicensed staff and oversight of the day to to day clinical needs in collaboration with the director of health services.

LPN Responsibilities (include not limited to):
- Administer nursing care to residents
- Establish and implementing treatment plans
- Monitor symptoms or behaviors in residents
- Communicate with collaborating physicians or Home Health/Hospice
- Educate residents and staff about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate resident records
- Provide advice and support to residents and their families
Qualifications:
- Previous experience in long term care preferred
- Familiarity with medical software and equipment
- Ability to build rapport with residents
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
- Active Idaho LPN license
Job Type: Full-time
Pay: $63,000.00 - $67,000.00 per year
