Position Overview
The Rapid Rehousing (RRH) Case Manager supports participants in the Gender Based Violence RRH program by facilitating their transition to safe and stable housing. This position focuses on identifying housing opportunities, managing rent subsidies, and addressing barriers to long-term housing stability through intensive case management. Regular in-person home visits ensure that clients receive the individualized support necessary to achieve their goals.
Key Responsibilities
- Conduct housing assessments and assist clients with identifying and securing appropriate housing.
- Develop individualized housing stability plans and provide ongoing case management to address barriers to housing retention.
- Revise and update housing stability plans at least every 6 months or based upon the client level of need.
- Conduct monthly in-person home visits to monitor progress, address challenges, and provide resources.
- Connect survivors to healthcare, mental health, legal support and other vital resources as needed.
- Maintain accurate client records and update required data elements in HMIS or alternate systems.
- Attend Accelerated Moving Events at various locations throughout the city.
- Complete case notes within 48 hours of service delivery and ensure all data entry is completed within 5 days
- Maintain complete and accurate case files, maintaining all files audit-ready.
- Participate in case conferences, SIT meetings, and required monthly RRH Partner Meetings with All Chicago
- Advocate and liaise with landlords, property managers and other staff on the participants behalf.
- Coordination with other service providers through client centered consultations.
- Create funding requests to ensure rent is paid on time.
- Ensure an exit pathway is in place for all participants in caseload.
- Maintain a caseload of 13 : 1
