Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing comprehensive case management services. This position involves assessing client needs, developing personalized care plans, and coordinating resources to ensure clients receive the necessary support. The Case Manager will work closely with clients from diverse backgrounds, particularly in areas related to behavioral health, childcare, and child welfare.
Responsibilities
- Conduct thorough assessments of clients' needs and circumstances.
- Develop and implement individualized care plans that address specific goals and challenges.
- Provide ongoing support and guidance to clients, including crisis management and motivational interviewing techniques.
- Collaborate with community resources and service providers to facilitate access to necessary services such as Section 8 housing assistance and Medicare.
- Monitor client progress and adjust care plans as needed to ensure optimal outcomes.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies.
- Participate in workforce development initiatives aimed at enhancing service delivery.
Qualifications
- Bachelor's degree in social work, psychology, or a related field preferred; relevant experience may be considered in lieu of a degree.
- Experience in behavioral health, childcare, or child welfare is highly desirable.
- Strong skills in crisis management and the ability to handle challenging situations with empathy and professionalism.
- Proficiency in motivational interviewing techniques to effectively engage clients.
- Excellent communication skills, both verbal and written, with the ability to build rapport with clients from diverse backgrounds.
- Knowledge of community resources related to housing assistance (Section 8) and healthcare (Medicare) is beneficial.
- Ability to work independently as well as part of a collaborative team environment.
