Position Summary

Family Service Coordinator is the member of the Infant and Toddler, Part C, early intervention team that is responsible for assessments, eligibility determination, IFSP development, implementation, ongoing assessment of services, and transition out of the program for children with developmental delays. Family Service Coordinators will complete the family interview portion of the evaluation as well as the vision screening. In some instances, the Family Service Coordinator will be required to complete additional areas of assessment during the evaluation process. Family Service Coordinators will maintain regular contact with families throughout the evaluation and eligibility process. Family Service Coordinators will be responsible for scheduling and facilitating the initial IFSP meeting as well as subsequent meetings throughout the family's time in the Part C program. Family Service Coordinators will complete monthly visits with all families on their caseload. Coordination service includes, but is not limited to, making appropriate referrals, advising families of their rights and procedural safeguards, conducting follow up activities, and facilitating the development of transition plans.

Supervision

Receive direct supervision from the STRIDE Learning Center Infant and Toddler, Part C, Assistant Director and general direction from the Executive Director.

Environmental Conditions

This position requires working indoors, in family’s homes, the majority of the time with occasional outside duties. Individuals must be able to withstand various noise levels from groups of children. Individual must be able to tolerate moderate levels of mental and emotional stress. Individual must be capable of assessing situations that may pose potential risks and respond with appropriate precautions. This position requires a valid driver’s license, valid vehicle insurance, and a reliable car.

Knowledge, Skills, and Abilities

  • Exercise sound judgment, creativity and sensitivity;
  • Demonstrate self-initiation in performing responsibilities and the ability to complete job functions without direct supervision;
  • Thorough working knowledge and understanding of IDEA Part C and eligibility of Part B;
  • Must be able to get on and off of the floor and move around with ease;
  • Must be able to lift 30 pounds independently and over 30 pounds with assistance

Required Education/Certification and Experience

This position requires a Bachelor’s, Master’s, or Doctorate Degree in social work, counseling, psychology or a related field. An Associate’s Degree in a related field combined with 10 years of experience in a related field will be acceptable. State licensure is also required as per state guidelines. Current CPR and First Aid certification is required. Three years of experience in early intervention, special education or a related field is preferred but not required.

The individual must pass a tuberculin (TB) risk assessment for employment. The individual must pass a child abuse/neglect Central Registry check and criminal background prescreen.

Specific essential duties/responsibilities the Family Services Coordinator based on experience, abilities, skill set, and need of the STRIDE agency.

50% Complete monthly visits with all families on caseload;

20% Conduct evaluations to determine eligibility and participate in initial IFSP development;

10% Enter initial IFSP data in the state required data base;

5% Schedule and facilitate quarterly group parenting classes;

5% Maintain all resources up to date and organized;

5% Participate in Part C staffing, all staff meetings, peer groups;

5% Conduct screenings.

All STRIDE Employees are expected to participate in fundraising activities as STRIDE is a non-profit 501(c)3 organization.

Job Type: Full-time

Pay: $22.00 - $38.00 per hour
