The Patient Coordinator is a vital role within our Diabetes RPM program, serving as the primary point of contact for patients. The coordinator supports patients by enrolling them in RPM programs, providing education, monitoring progress, and collaborating with medical teams to ensure exceptional care. This role combines administrative, educational, and support functions to enhance patient engagement and outcomes. Key Responsibilities
- Patient Enrollment and Engagement
Enroll individuals with diabetes into RPM programs, ensuring they understand program benefits and processes.
Schedule and coordinate follow-up appointments with physicians through EMR systems or warm transfers.
Maintain accurate documentation of all patient interactions in electronic health records (EHRs).
Collaborate with medical professionals, including endocrinologists and diabetes educators, to deliver holistic care.
