- JOB DUTIES
- Assess home health pediatric patient for rehabilitative and functional status.
- Regularly re-evaluate the patient’s / client’s physical therapy need and evaluate the outcomes of care on a regular basis.
- Physical Therapist develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care.
- Identifies deficits including but not limited to muscle strength / coordination, mobility, gait, ROM and potential for intervention. Evaluates the home environment.
- Physical Therapist evaluates the need for and recommends DME devices and aides.
- Uses infection control measures that protect both the staff and the patient (OSHA).
- Coordinates services. Informs the physician and other personnel of changes in the patient's / client’s condition and needs.
- Physical Therapist demonstrates commitment and professional growth by participating in in-service programs and maintaining or improving competency.
- Supervises, teaches, and provides clinical direction to other physical therapy personnel as required or requested.
- Promote Agency philosophy and administrative policies to ensure quality of care.
- Physical Therapist provides effective communication to patients / clients, their family members, team members, and other health care professionals.
- QUALIFICATIONS
- Graduate of an accredited Diploma, with Bachelor’s, Master’s, or Doctorate degree
- One year of experience as a Physical Therapist in a pediatric clinical care setting preferred
- Pediatric Home health experience preferred
- Reliable transportation
- Current Texas State license as a Physical Therapist
