**Key Responsibilities**
As a RN Care Manager, your key responsibilities will include:
- Care Plan Development: Develop thorough care plans addressing clinical and non-clinical needs of each patient.
- Assessment and Evaluation: Conduct on-site and virtual assessments to detect undisclosed medical or social issues.
- Outreach and Engagement: Proactively contact patients and/or clinics to evaluate their status and offer assistance.
- Patient Panel Management: Manage a diverse patient panel, evaluating individual needs while adhering to NCQA standards.
- Care Team Collaboration: Communicate updates to the care team and family members, delivering tailored education and resources.
**Requirements**
- Nursing Education: Associate's degree from an accredited nursing school.
- Licensure: Current Registered Nurse (RN) license in the state of practice.
- Experience: 3 years combined experience in nephrology, ER, ICU, Cardiac, or Ambulatory Complex case management nursing preferred.
- Certification: Certification as a Certified Case Manager (CCM) is also preferred.
