Job Summary:
We are seeking a full-time Nurse Practitioner to provide psychiatric evaluations, medication management, and follow-up care for residents in nursing homes and assisted living settings. You’ll be supported by a full-time medical director, administrative staff, and outsourced billing services — so you can focus on patient care.
Responsibilities:
- Conduct initial psychiatric assessments and routine follow-up visits
- Prescribe and manage psychotropic medications
- Collaborate with facility staff, medical directors, and care teams
- Document in EMR in a timely and accurate manner

Nurse Practitioner Job Qualifications:
- Must have a current and active NP license and DEA # in the State of Wisconsin.
- Must have at least one year experience working as a Nurse Practitioner.
- Be willing and able to commute to long term care facilities.
- Experience with geriatric populations or interest in working with older adults
- Compassionate, patient-centered care philosophy
- Strong communication and collaboration skills

Pay: Competitive base productivity bonus (commensurate with experience)
