Stubbe Case Management is seeking an RN Field Nurse Case Manager to coordinate and manage the medical needs of individuals/clients who have sustained a work-related injury. The Nurse Case Manager uses their clinical knowledge, expertise, and leadership skills to partner with all stakeholders to facilitate the best possible outcome for the injured worker.
This is a full-time, benefits-eligible position with flexible scheduling options of 25, 30, 35, or 40 hours per week, based on your preference and experience. This is also a hybrid position comprised of 2/3 field travel in the Milwaukee/Madison, WI area and 1/3 work from home office.

A Day in The Life of an RN Field Case Manager
Traveling within the Milwaukee & Madison, WI service areas to attend medical appointments and obtain clinical information including diagnosis, treatment, and return to work plans.
Partnering with medical providers and therapists to obtain clinical updates at in-person medical appointments.
Completing daily documentation of emails and reports outlining medical updates and case developments.
Working with insurance companies, employers, attorneys, or any other stakeholder who has direct involvement in the workers compensation claim, via phone, to stay informed of the status and progression of injury recovery.
Reviewing medical records to appropriately understand and summarize medical care being provided.
Coordinating ongoing appointment and diagnostic needs, as determined by the medical provider.
Facilitating return to work plans with the employer, employee, and medical provider.
Formulating, reevaluating, and updating injured worker/client focused goals to measure progression toward recovery and to optimize best outcomes.
What You Will Need to Be Successful
An active RN licensure in the State of Wisconsin.
Case management experience is strongly preferred.
Workers compensation industry experience is preferred.
Computer proficiency and technical aptitude to use Microsoft Office and to learn a case management technology system.
Excellent written, verbal, and interpersonal communication skills.
Aptitude for working both independently and within a team environment.
An ability to advocate, educate, and be assertive to ensure the best possible result for the injured worker.
Strong time management skills, including being able to effectively prioritize and adapt to a dynamic, fluctuating caseload.
