Sturdy Health is seeking a passionate and dedicated Community Health Navigator to support our Maternal Equity Program funded by the CHSF Grant. This role focuses on improving health access and equity in maternal care by addressing barriers to care and connecting expectant and new mothers with essential resources. The Navigator will work closely with at-risk birthing populations, ensuring culturally competent care and fostering a supportive network to improve maternal and neonatal outcomes.
Education / Training
- Experience in community health, patient advocacy, or maternal health programs.
- Strong understanding of social determinants of health and cultural competence.
- Excellent interpersonal and communication skills.
- Bachelor's degree in public health, Social Work, Nursing, or a related field preferred.

Essential Job Functions :
Assist expectant and new mothers in navigating the healthcare system, including prenatal, delivery, and postpartum care.
Work collaboratively with translation services to ensure linguistic needs are met.
Coordinate access to maternal health resources, including remote monitoring for diabetes, weight, and blood pressure.
Deliver educational programs on maternal health topics, such as gestational diabetes, nutrition, and postpartum care.
Conduct outreach to at-risk populations and build relationships with local community organizations.
Track and report patient outcomes using EMR systems to assess the effectiveness of programs.
