GENERAL DESCRIPTION OF POSITION:
The Behavioral Health Licensed Masters Social Worker, Care Manager, under the clinical supervision of an LCSW, will provide specialized social services to the School Based Clinic and/or Su Clinica sites to include assessments, interventions, therapeutic modalities, and consultation as it relates to behavioral health and behaviorally influenced conditions. The BHCM will be a part of the Behavioral Wellness Team, attending to patients of all ages and wide-ranging medical/behavioral conditions. The BHCM will review and reach out to referrals provided to them by Su Clinica Providers to assess for urgency, SUDS, psychiatry, and depression/anxiety.
The BHCM will conduct a brief intervention assessment. The BHCM will provide behavioral health services and interventions to a high volume of patients. They will be available to assist and support the counselors with members facing complex medical/behavioral health issues including non-adherence to medications/medical advice, or multiple complex co-morbidities. The BHCM will demonstrate clinical knowledge and skills to assess and treat mental, emotional, and behavioral disorders in adults, adolescents, and children. The BHCM will apply therapeutic modalities and methods to the treatment and prevention of psychosocial dysfunction, disability, or impairment, including emotional and mental disorders. The treatment interventions utilized, such as behavioral and cognitive behavioral modalities, solution focused brief therapy, and motivational interviewing and will assist SC patients in behavior change to maintain a healthy lifestyle and quality of life.
They will perform case management duties in assisting members and their families with full utilization of community resources, getting them connected and following up with outside referrals made. They will work integrated with Adult and Pediatric Psychiatrist by connecting patients with services, assisting with prescription refills, and coordinating psychiatric care. As a member of the clinical team, this position requires total support to the PCMH principals, policies, and procedures. The BHCM will be available to the Behavioral Wellness department to coordinate patient care within the interdisciplinary team. The BHCM will abide by Su Clinica policies and procedures as well as with the policies and procedures of the school district.
ESSENTIAL JOB FUNCTIONS: (With or without accommodations)
Patient Relationship: Conducts brief patient psycho-social assessment and directly address issues such as physical, mental, financial, domestic, and immediately any life-threatening domestic violence. Communicates with empathy with the patients and their families and provides emotional support to patients and arrange required services through the referral process.
Management of Patients: Services involve frequent outreach and assistance developing and working to meet defined Behavioral healthcare goals. Report situations identified directly to CPS (Child Protective Services) or APS (Adult Protective Services) according to state regulations. Prepares monthly reports or participates in pilot programs as requested. Conducts crisis screening and evaluation; works with general clinic providers to assure patient safety. Administer mental health screening tools, such as PHQ-9; GAD-7; DAST; CRAFFT; CSSR-S,Vanderbilt; ASQ, etc. Expand counseling services provided to patients and their families to include utilization of brief therapeutic and supportive interventions, enhance mental health screenings, assessment, and support. Adheres to all HIPAA regulations in and outside of the Clinic. Communicates patient information as requested to enhance patient services.
Team Communication: Functions as a vital member of the Behavioral Health care interdisciplinary clinical team. Serves as the clinic’s consultant in the identification and coordination of community resources and SC’s Behavioral Health patients. Approaches patient care as a “team” involving clinical and non-clinical staff (eg. Physicians, Psychiatrists, Mid-levels, Behavioral wellness counselors, educators, appointments staff) to achieve patient care objectives. Participates in team conferences, peer reviews, participates in educational sessions for peer development and increase knowledge of community resources. Submits required documentation in a timely manner (leave requests, credentialing requirements, license renewals, certifications, CEU attendance, etc.).

Refrains from texting or speaking on cell phone while on duty. Refrains from informal communications with patients and others. Fully participates in the performance improvement and follows all the Clinic policies and procedures. Attends work on a regular and predictable schedule in accordance with clinic leave policy and performs other duties assigned.
Qualifications

Knowledge of PCMH Model
Knowledge of modern social workers’ practices and methods. Know of electronic medical records.
Knowledge of and adherence to existing professional ethical standards.

Knowledge of computer software (Microsoft Word, Excel, etc.) programs.
Knowledge of Service Coding and Treatment.

Ability to submit documentation timely and in adherence to service coding and treatment.
MINIMUM QUALIFICATIONS/CERTIFICATES/LICENSES/REGISTRATIONS REQUIRED:
Must have Masters in Social Work from an Accredited College/University and be licensed by the Texas State Board of Social Worker Examiners. Must be current with BLS/CPR. Bilingual (English/Spanish) required.
SPECIAL INSTRUCTIONS: May require contact with individuals with communicable diseases.
Due to the nature of SC Services, it may be necessary for employees to work extended hours or other variations of the usual shift to ensure adequate care to patients and to maintain service to the community.
