SUMMARY OF DUTIES :
18.70 - $19.45 / hour
The Residential Supervisor is responsible for scheduling, training and evaluating the performance of assigned direct support staff. The supervisor provides direct care service to residents as assigned; and monitors implementation and documentation of habilitation plans. Other key areas of responsibility include maintaining a clean, comfortable and safe home environment, and personal allowance.
REPORTING RELATIONSHIP :
Receives supervision from the Residence Manager / Team Leader.
Essential Duties :
1. Schedules, trains and evaluates direct support staff. Ensures training is current, and completes new employee site orientation.
2. Provides direct service to residents as assigned and / or needed.
3. Reviews site documentation within specified time frames (i.e. communication logs, incidents, Illness, Injury, vehicle checklists, maintenance and purchase requests) to insure timely follow up.
4. Responsible for staff implementation and documentation of the habilitation plan.
5. Ensures documentation is thorough, accurate, contemporaneous; and in compliance with billing standards. Completes attendance / billing records for timely submission.
6. Responsible for the security of personal allowance funds and documentation of related transactions. Ensures availability of funds for residents in a timely manner. Monitors for the completion of a personal inventory.
7. Facilitates staff meetings. Records and maintains minutes.
8. Responsible for ensuring that equipment, supplies and furnishings within the home is appropriate according to the needs of the residents. Completes purchase requests and makes purchases timely.
9. Responsible for ensuring the cleanliness and maintenance of the home and outside area. Ensures work orders are submitted with timely resolution.
- Any identified health or safety issues will be addressed immediately.
10. Records and dispenses medications in accordance with medication administration procedures.
11. Reviews medical appointment records, Physician’s Notes and Orders and completes any necessary medical follow up.
12. Schedules and maintains fire drill records for the assigned site. Ensures staff review of fire evacuation plan, Site Plan of Protection, and Individual POPs every other month or as changes occur.
13. Responsible for the coordination of menu planning, shopping, inventory and purchase of food.
14. Reviews monthly recreation calendar and community activity plans. Schedules supports for activities accordingly. Encourages and supports residents to express their opinions, suggestions for improvement via a monthly meeting.
15. Participates in quality surveys and activities as scheduled.
16. Maintains regular and positive communication with individuals served and stakeholders.
17. Completes other duties as assigned.
QUALIFICATIONS :
High School Diploma or GED PLUS one (1) year human service experience. Valid New York State driver’s license with acceptable driving record within Agency’s criteria.
LANGUAGE SKILLS :
Provide effective, two-way communication with people, both orally and in writing, and to understand communication from others in verbal and written format.
Ability to read and interpret a variety of documents / instructions furnished in written, oral, or schedule form.
Ability to effectively present information informally in one-on-one or small group situations.
MATHEMATICAL SKILLS :
Able to perform basic math functions (i.e., addition, subtraction, multiplication, and division, and percentages)
REASONING ABILITY :
Be able to solve most daily problems and to obtain the information necessary to solve more complex problems
CERTIFICATES / LICENSES & REGISTRATIONS :
Must be Medication, CPR / First Aid and SCIP-R certified.
COMPUTER SKILLS :
Ability to use Microsoft Word for development of reports, memos, letters, etc.
Ability to use Microsoft Outlook for email, task assignments, scheduling meetings etc.
Ability to use all functions in PrecisionCare
This position may require access to other computer systems.
PHYSICAL DEMANDS :
Must be able to perform essential duties as described on attached form (checked areas are conditions or duties that you will either be exposed to or are expectations of the agency.)
TRAINING REQUIREMENTS :
Must complete all mandatory agency training within the initial agency orientation period.
ATYPICAL EMPLOYMENT CONDITIONS
Able to work a variety of hours, address unusual or emergency situations that may include weekend and / or holiday timeframes.
Able to fulfill responsibilities associated with Agency’s on-call system
