The Hospice Licensed Practical Nurse (LPN) provides nursing care under the direct supervision of a Registered Nurse (RN) in accordance with the interdisciplinary plan of care.
- Provides nursing services as directed by the Plan of Care including, but not limited to: wound care, medication administration and education and teaching to patient and family members.
- Completes documentation in accordance with agency expectations and maintains consistent communication with the supervising RN regarding patient care.
- Demonstrates a commitment to patient’s first through excellent customer service.
- Demonstrates critical thinking, analysis, and clinical judgment in the care of patients/families experiencing life-limiting, progressive illness through the use of the nursing process to address the physical, emotional, psychosocial, and spiritual needs of the patients/ families.
- Incorporates ethical principles and professional standards in the care of, as well as identifying and advocating for the wishes and preferences of patients and families.
- Actively promotes dialogue with patients and families, the healthcare team and community to address and plan for issues related to living with and dying from life limiting and progressive illness.
- Utilizes resources necessary to enhance quality of life for patients/ families through knowledge and negotiation of the healthcare system.
- Demonstrates cultural competence.
- Facilitates education for families, patients, members of the healthcare team and community about dying from life limiting illness.
- Demonstrates the use of effective communication.
- Actively participates as a member of the interdisciplinary team.
- Demonstrates knowledge, attitudes, behaviors, and skills that are consistent with the professional standards, code of ethics and scope of practice for hospice nursing.
- Participates in on-call duties, as assigned
- Must participate in mandatory continuing education as required by regulatory, accreditation bodies, scope of practice, and/or Suburban Hospice
Basic Qualifications:
- Must be a Licensed Practice Nurse (LPN) by the Indiana Professional Licensing Board
- Must have at least 1 year experience as a LPN.
- Previous experience in hospice or home health preferred.
- Must possess valid driver's license and automobile insurance.
- Ability to manage and report data in a timely fashion, facilitate problem resolution, communicate effectively in the English language and be an interdisciplinary team participant.
