Responsibilities

• Conduct thorough patient assessments, including physical examinations and health histories.
• Develop and implement individualized care plans based on patient needs.
• Provide primary care services in areas such as internal medicine and palliative care.
• Manage case management responsibilities, ensuring continuity of care for patients.
• Perform triage duties to prioritize patient needs effectively.
• Educate patients and families on health management and disease prevention strategies.
• Maintain accurate medical records using electronic health record systems.
• Participate in quality improvement initiatives and contribute to the development of clinical protocols.

Experience

• A Master's degree in Nursing or equivalent from an accredited program.
• Current Nurse Practitioner certification with relevant state licensure.
• Experience in palliative medicine or sterile processing is a plus.
• Familiarity with case management practices and triage protocols is preferred.
• Strong communication skills with the ability to educate patients effectively.
