About the Position
We are seeking a skilled Licensed Practical Nurse to join our team at Sullivan County Health Care. This is a per diem position that requires flexibility and a commitment to delivering high-quality patient care.
Responsibilities
- Administer medications and treatments as ordered by physicians
- Assess residents' mental and physical well-being, developing and updating care plans as needed
- Supervise Licensed Nursing Assistants (LNAs) on assigned units, assigning and measuring nursing staff resident care consistent with abilities and job description
- Accompany physicians on rounds, addressing residents' medical needs and facilitating communication during 60-day reviews
- Perform regular unit duties, such as checking appointments, monitoring LNA documentation, and ensuring infection control practices
- Admit, discharge, and transfer residents, completing necessary documentation and notifications
Requirements
- A current license in the State of New Hampshire as a Licensed Practical Nurse without restrictions or sanctions of any kind
- A graduate of a School of Nursing program accredited by the American Nurses Association
- One to three years' experience in an acute or long-term care facility preferred
- Certification for CPR and may be required to obtain certification to administer IV therapy
Work Schedule
This per diem position requires working one full weekend per month and at least five holidays per year. Shift differential pay applies for second and third shifts, and an additional 20% of base rate of pay is provided for all weekend shifts.
