Responsibilities:
- Attend all scheduled shifts on time and follow Sullivan County's attendance policy.
- Accurately receive, transcribe, and carry out physician orders.
- Administer medications and treatments as ordered by the physician and maintain proper documentation and notifications.
- Assess residents, develop and update care plans, MDSs, and quarterly assessments within mandated time frames.
- Supervise licensed nurse assistants (LNAs) under New Hampshire Board of Nursing licensing requirements.
- Assign and measure nursing staff resident care consistent with abilities, job description, and scope of practice.
- Perform wound care, administer oxygen, and gastrostomy/jejunostomy feedings according to facility guidelines and procedures.
- Accompany physicians on resident rounds, address medical needs, and facilitate communication during 60-day reviews.
- Complete 24-hour reports, check new orders, and maintain consistent communication with the nursing supervisor or unit manager.
- Communicate with residents' families to resolve concerns and notify family members of new orders.
Qualifications:
- A current license in the State of New Hampshire as a Licensed Practical Nurse without restrictions or sanctions.
- A graduate of a School of Nursing program accredited by the American Nurses Association.
- One to three years' experience in an Acute or Long Term Care facility preferred.
