What are the responsibilities and job description for the Home Health Care Scheduler position at Sully Home Care Services?
Duties:
- Create and manage direct care visit schedules daily/monthly as changes are required
- Monitor visits and make sure that they are scheduled appropriately and in a timely manner
- Review list of screened aides to find coverage when an aide is not able to work
- Communicate with clients and aides to confirm schedules
- Coordinate with clinical team, HR, and others in the agency to ensure continuity of care
- Document missed visits
- Perform other office duties as required
Requirements:
- Minimum H.S. diploma or GED and one year of experience in home care or office environment
- Prior scheduling experience preferred
- Strong computer skills
- Excellent communication and interpersonal skills
- Maturity and ability to deal effectively with the demands of the job
As a Home Care Scheduler your primary function is to schedule and manage the flow of patient care, ensuring that quality service is provided and patient satisfaction is met. The scheduler provides the scheduling and coordination of the daily schedules for the direct care staff matching the schedule with staff availability and suitability.
Job Type: Full-time
Pay: $18.95 - $25.00 per hour
Expected hours: 40 per week
