What are the responsibilities and job description for the Diabetes Care Educator - Endocrinology Office - PRN position at Summa Health?
Collaborates with members of the health care team to provide diabetes education or inpatients and outpatients. Continuously evaluates and monitors the effectiveness and outcomes of Diabetes Self-Management education (DSME) intervention. Provides diabetes education to patient and family members. Fosters continuity of care among all phases of the system. Participates in performance improvement planning. Actively assesses, evaluates and maintains professional development and competency.
Formal Education Required:
a. Registered Nurse, Registered Dietitian, or Master Certified Health Education Specialist (MCHES)
b. Certified Diabetes Care and Education Specialist (CDCES) certification and Member of the American Diabetes Association
Experience & Training Required:
a. One (1) year of diabetes education experience
Other Skills, Competencies and Qualifications:
a. Ability to operate a computer
b. Population Specific Competency: Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity
Level of Physical Demands:
a. Sedentary: Exerts up to ten pounds of force occasionally and/or a negligible amount of force frequently
