Description

Position Summary: 

The Behavioral Health Provider acts as an independent clinician who provides behavioral health counseling to children, adults, and families in a clinic, or a school-based healthcare setting as assigned by BHSLD. The behavioral health provider works closely with primary care providers, dental providers, psychiatry, and school systems to treat behavioral health conditions while upholding the ASQ (Access, Stewardship, and Quality) pillars. As a part of the primary care team, the Behavioral Health Provider will provide comprehensive diagnostic and behavioral health services, including brief assessments, interventions, psychoeducation, referrals, and consultations for patients in school-based health clinic settings. Behavioral Health Providers work from a systems and community perspective to formulate treatment conceptualizations, interventions, and case management goals.

 

Behavioral Health Providers will develop treatment plans with their patients/family system and care teams and utilize evidence-based psychosocial interventions to achieve treatment goals. Behavioral Health providers may use several therapeutic modalities in this clinical setting, including play therapy, individual therapy, group therapy, and case management. The Behavioral Health Provider must demonstrate regard for the rights of all patients and provide respectful and considerate care focused on the patient's needs.

Essential Functions:

  • Maintains a visible presence to providers during clinic operating hours, providing same-day verbal or documented feedback on patient encounters in person or by phone.
  • Is available for "curbside" consultation (a brief interaction) by being in the clinic or available by phone or Teams and is available for same-day and scheduled initial consultations with patients referred by other service lines.
  • Provide clinical consultation and serve as a resource for colleagues in the behavioral health department and broader community.
  • Promote integrated care by engaging medical providers and other clinical staff to facilitate teamwork and improve patient care processes and outcomes.
  • Conduct outreach to patients around scheduling, response to medication, and other case management needs.
  • Provides brief interventions for patients, performing short, limited follow-up visits for selected patients.
  • Provides a range of services, including screening for common conditions, assessments, and interventions related to chronic disease management programs.
  • Conducts risk assessments as indicated.
  • Provides psychoeducation for patients during individual visits.
  • Identifies, reviews, and maintains an up-to-date library of patient education materials for commonly seen problems for accuracy, literacy level, and cultural appropriateness.
  • Triage patients with severe or high-risk behavioral problems and refer them to resources consistent with the level of care criteria.
  • Documents should be included in the EHR consultant notes explaining assessment findings, interventions delivered, and recommendations.
  • Maintains clinical records and other necessary paperwork in a timely manner to comply with all administrative regulations.
  • Assist in capturing program evaluation and fidelity measures.
  • Attend clinic meetings, supervisions, department meetings, all staff meetings, and other meetings assigned.
  • Participate in program development through quality initiatives and discussions with team members and supervisors.
  • Work with care teams and providers to increase their understanding of mental health issues with patients.
  • Collaborate with necessary entities to provide aftercare planning and treatment following hospitalization or a higher level of care.

 

Additional Requirements:

  • Grants are vital to Care Clinic revenues and directly or indirectly support all positions. This role may have responsibilities that are closely associated with grant deliverables. Grants and grant responsibilities may change over Time.
  • Complete all the required training as designated by HR and the position supervisor.
  • Oversight of projects and tasks assigned by the COO, Dental Director, and Dental QI Director.
  • Adhere to all policies within the organization and those outlined within the SCCC Employee Handbook.
  • Comply with all SCCC Vaccine Mandates.
  • Willingness to travel and work at other SCCC locations as determined by the supervisor.
  • Comply with all applicable Credentialing and Privileging requirements in a timely manner.

  • Must participate in quality improvement activities when applicable.

Qualifications


Qualifications:

Education/Experience:

 

Must Haves to be successful:

  • Masters level degree or higher in one of the recognized mental health professions (counseling, social work, marriage and family therapy). M.A., M.S. in Clinical Counseling (or related area), LSW, LCSW, LPC, LMFT. Being licensed in Colorado is preferred, and being license-eligible is considered.
  • Registered with the Department of Regulatory Agencies (DORA).
  • Knowledge in lifespan development and experience in assessing, diagnosing, and treating mental health conditions in adult populations.
  • Academic and/or job experience in legal and ethical decision-making, individual and cultural diversity, diagnosis and treatment planning, evidence-based practice and interventions for psychotherapy, theoretical and conceptualization skills, psychopharmacology and behavioral medicine, professional consultation, integrated care, clinical administration, and utilization of electronic health records.
  • Reading, writing, and clearly speaking the English language is required. 
  • Read, analyze, and interpret complex documents.
  • Knowledge of word processing software, spreadsheet software, and database software.
  • Ability to work in a team and to share responsibilities and duties. 
  • Ability to resolve interpersonal conflict in a straightforward, empathic, and timely manner. 
  • Ability to be friendly and empathic with co-workers and patients.

 

Nice to Haves:

  • Being licensed in Colorado is preferred, and being license-eligible is considered.
  • Experience in evidence-based therapies is preferred (e.g., EMDR, DBT, CBT, etc.).
  • Knowledge and experience in psychopharmacology and behavioral health medicine are a plus, and willingness to grow in these areas is required. 
  • Two years of directly related experience is preferred.
  • Couples and family therapy experience is a plus.
  • Reading, writing, and clearly speaking the Spanish language is preferred.

 

Work Environment: 

The work environment in a healthcare setting can be fast-paced and involves working directly with patients and healthcare professionals. The environment is primarily a clinical setting, but the individual may also work in a clerical setting.
