This position is responsible for the development of a person-centered plan for individuals that promotes self-determination. Assists individuals and their families/guardians to explore and obtain services and supports from a variety of sources. Assures that services delivered are in compliance with federal, state, local rules, regulations, statutes, standards, as well as Board policy, regulations and procedures and related Operating Plans. Must strive to meet the Agency mission and exemplify the Agency’s core values.
Minimum Qualifications:
- Education: Bachelor's Degree in Human Services, Business Administration, or related field.
- Experience: A minimum of three (3) years’ experience in coordinating services in DD or related human services field.
- Skills: Effectively communicate both orally and in writing. Perform basic math skills. Maintain accurate records.

Knowledge, Abilities, and Other Expectations:
- Knowledge: Proficient with computers and use of applicable software (MS Word/Outlook), ability to use general office equipment. Must have good working knowledge and understanding of behavior support, individualized service plans, person-centered planning, medical, psychosocial, orthopedic, and psychiatric aspects of disability, client rights, waivers, benefits, self-determination, HIPAA, and House Bill 94. Knowledgeable of applicable statutes, regulatory standards, and reporting authorities. Good working knowledge of DD programs, services and community resources.
- Abilities: Must be skilled in group facilitation and/or mediation, conflict-resolution, crisis intervention, and working through complex problems situations. Effectively manage time to meet operational needs and desired outcomes and plan and organize the steps to implement new projects. Listen to, understand and communicate information and ideas. Work independently with minimal supervision. Maintain strict confidentiality. Work as part of a team and work effectively with persons and communities from diverse cultural, social, and ethnic backgrounds. Work effectively under pressure.
- Other Expectations: Report unusual incidents and major unusual incidents. Complete all required training and maintains strict confidentiality. Regular attendance is mandatory. Maintain related records in accordance with appropriate records retention guidelines and policies. Adhere to health and safety policies and procedures. Must have reliable transportation to fulfill responsibilities requiring travel in the community. Must be available by personal cell phone during working hours.
Evaluation of this position is based primarily on performance of the following essential functions, which include, but are not limited to:
1. Coordinates the development and continuous review of the person-centered assessment and individual service plan (ISP) to ensure the ISP is designed to empower each individual to contribute to his/her own success and to that of his/her community using the team planning process.
- Assists individuals/families with identifying, selecting, and obtaining services/supports that are important to and for the individual. Assist individuals/families in connecting with community resources.
- Coordinates and develops an assessment and ISP with the person and his/her team that supports the concepts and principles of individual rights, well-being, personal development, interpersonal relationships, social inclusion, and self-determination including individual choice of service providers, development of individual budgets and personal resources, and individual control of services to the extent feasible for each individual.
- Assists with provider selection and authorization of services within the individual’s budget following all applicable rules and parameters.
- Coordinates a continuous review process to monitor individuals identified services and outcomes to ensure services are being delivered according to the ISP. Provides feedback regarding the results of review to individual/family and other appropriate parties and updates and makes changes to the ISP as applicable.
2. While supporting the mission and vision of Summit DD, works collaboratively with administrators, co-workers, other agencies, professionals, families and the public in ways that enhance and support individuals served and person-centered thinking.
3. Assists individuals with increasing self-advocacy, and/or identifying a chosen representative. Explains rights and informed consent for services in order to promote self-determination.
4. Encourages meaningful community involvement. Collaborates with and maintains open and frequent communication with community resources to develop natural supports and opportunities.
5. In accordance with DODD and Summit DD requirements, documents and submits Medicaid billing documentation in accordance with the Targeted Case Management and / or Medicaid Administrative Claiming Guide.
6. Complies with guidelines established by DODD and Summit DD regarding reporting alleged abuse and/or neglect and other incidents as outlined in Ohio Administrative Code Rule 5123:1-17-02: Major Unusual Incidents. Assures an appropriate response to situations that demonstrate a threat to health and/or safety.
7. Complies with all local, state, federal rules, regulations, statutes, and standards, including but not limited to Summit DD interpretations of said rules and with all regulatory standards such as state and federal law, Title XX, Title XIX, CARF standards, and the Ohio Administrative Code.
8. Attends all required training; identifies any training needs, and requests training and support.
9. Performs other related duties as assigned.
