Job Summary:
Summit Healthcare Group is seeking a dedicated and experienced Regional Social Services Director/Activities Consultant to oversee and guide the Social Services and Activities departments across multiple Skilled Nursing Facilities. This role will focus on ensuring that residents' psychosocial, mental, and emotional needs are met, while also fostering an environment that maximizes their individuality, independence, and dignity. The position will involve providing facility staff with the tools to ensure that residents receive the highest practical level of physical, mental, and psychosocial well-being and quality of life. Key Responsibilities:
Provide support and guidance to facility-level Social Service Directors and Activity Directors in identifying and meeting residents' psychosocial, mental, and emotional needs.
Ensure that residents receive the highest level of physical, mental, and psychosocial well-being through personalized care plans.
Assist with the development and implementation of programs that support residents’ emotional and psychological needs.
Oversee Social Services and Activities staff, ensuring that all psychosocial and emotional needs of residents are met consistently.
Provide tools, training, and guidance to facility staff to promote a climate of dignity, individuality, and independence for residents.
Assist with the MDS process and ensure social service assessments are properly completed and updated.
Foster a team-oriented environment and contribute to a positive workplace culture that prioritizes collaboration and respect.
Ensure that all services are in compliance with state and federal regulations and organizational standards.
Qualifications:
2 years of social services consulting experience in a Skilled Nursing or Long-Term Care setting preferred.
Proven experience managing Social Services departments, including understanding and implementing the MDS process and social service assessments.
Desire to be part of a team that works, grows, and excels together.
Strong leadership and communication skills with the ability to work effectively with interdisciplinary teams.
Ability to foster a positive and supportive environment for both residents and staff.
Knowledge of regulatory requirements and best practices in long-term care.
