Essential Functions / Major Responsibilities:
- Perform patient evaluations, formulate plans, provide direct patient care and supervise Therapy Services Techs and Therapy Assistants within scope of practice.
- Appropriately document patient progress on a daily basis.
- Appropriately document patient evaluations and progress for all patients.
- Send initial evaluations and progress reports in a timely fashion.
- Complete all duties of a Therapy Services Tech and Therapy Assistant within scope of practice
- Attend in-service and continuing education.
- Attend other meetings per choice of department director.
- Help in implementation of policies and procedures.
- Address pediatric, adolescent, adult and geriatric patient and family concerns, demonstrating specific knowledge of the needs, development and care and/or treatment of the pediatric, adolescent, adult and geriatric patient population.
- Complete continuing competence for 2 year renewal period.
- Notify Board with name and address change within 30 days.
- Adhere to applicable State of Arizona licensure statutes and applicable national standards such as but not limited to NBCOT, ASHA as required to maintain professional designation.
- Displays proper etiquette and mannerisms that reflect the Shine Behavior Standards.
- Promotes the Patient Safety Standards as a core value of the organization.

Education and/or Experience:
- Graduated of an accredited Physical Therapy program (required).
- Licensed in the state of Arizona (required).
- High School diploma or equivalent (required).
- Current CPR/BLS certification (required within 30 days of hire date). Employee to maintain certification without gaps in order to maintain Success Sharing eligibility. Suspension without pay will occur 31 days after expiration.

Exert up to 50lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects. The worker is subject to contact with chemicals, contact with infectious fluids, heavy lifting, constant standing, and encounters with upset/disturbed individuals.
Physical motions include finger dexterity, standing, walking, stooping, crawling, talking, reaching, feeling, sitting, bending, kneeling, climbing, grasping, listening/hearing, handling, lifting up to 50-100 pounds, and repetitive motions of the hands, wrists, and feet.
