The Assisted & Independent Living Account Manager is responsible for maintaining business relationships and coordinating referrals from Senior Living Communities and other related customers.

          Coordinate and initiate patient referrals from Assisted & Independent Living communities, including obtaining patient/POA consent, communicating with community orders acquisition support, and coordinating fac

          Schedule regular stops with assigned accounts and attend regular care conferences with clinical
        • teams and buildins.
          Provide feedback to ADON and Rehab coordinators regarding operations and clinical concer
