Education :
Bachelor’s degree in healthcare or business-related field preferred.
Experience
Minimum 1 years sales experience, at least one in home health, hospice, or other post-acute
healthcare area.

- Client Relationship Management:
- Build and maintain strong relationships with key stakeholders in Assisted and Independent Living communities.
- Develop relationships with administration and clinical teams within facilities.
- Maintain communication with patients, families, Assisted & Independent Living buildings, and associated clinicians within assigned accounts.
- Ascertain rules of engagement with Assisted & Independent Living buildings and communicate them to clinicians.
- Lead client satisfaction initiatives and gather feedback to continuously improve service delivery.
- Business Development:
- Identify and pursue new business opportunities within the assigned territory.
- Generate referrals from facilities and communities.
- Collaborate with and support business development efforts of assigned accounts.
- Develop and implement innovative strategies to expand market presence and drive growth.
- Account Management:
- Oversee the day-to-day management of client accounts, ensuring high levels of client satisfaction and retention.
- Coordinate and initiate patient referrals from Assisted & Independent Living communities, including obtaining patient/POA consent, communicating with community orders acquisition support, and coordinating face-to-face visits.
- Maintain tracking of referrals with appropriate reporting.
- Schedule regular stops with assigned accounts and attend regular care conferences with clinical teams and buildings.
- Provide feedback to ADON and Rehab coordinators regarding operations and clinical concerns noted in facilities.
- Conduct joint, in-person calls with direct reports.
- Ensure that quality and timeliness standards are achieved by their team.
- Develop and implement best practices for account management to enhance efficiency and effectiveness.
- Sales and Marketing:
- Collaborate with the sales and marketing teams to develop and implement strategies to promote our services.
- Participate in regular strategy meetings with the Director of Assisted & Independent Living and execute action steps.
- Develop and execute value-added programming with Rehab coordinators, clinical teams, and Assisted & Independent Living buildings.
- Create and deliver compelling presentations to potential clients and stakeholders.
- Service Coordination:
- Work closely with clinical and operational teams to ensure seamless delivery of services to clients.
- Coordinate standardized communication processes with clinicians and Assisted & Independent Living buildings.
- Communicate with DON/ADON and Director of Rehabilitation about care issues and case management.
- Facilitate training and development programs for clinical and operational teams to enhance service delivery.
- Reporting and Analysis:
- Monitor account performance, prepare regular reports, and provide insights to improve service delivery and client satisfaction.
- Utilize data analytics to identify trends and opportunities for improvement.
- Compliance:
- Ensure all activities comply with relevant regulations and company policies.
- Maintain confidentiality in all aspects of patient care.
