What are the responsibilities and job description for the RN Supervisor for Home Care Services position at Summit Home Care New Jersey?
Summit Home Care is a family-owned and operated home care agency. We are currently looking for compassionate and reliable per diem RN Supervisors to join our team of dedicated professionals. The RN supervisor is responsible for the oversight of their patient from admission through discharge.
Responsibilities :
- Works with Summit Patient Coordinators to ensure accurate clinical documentation for case opening and ongoing status of patient.
- Opens cases and initiates the CHHA plan of care and competency supervision
- Conducts Bi-monthly client assessments in-home and virtually as needed
- Is the liaison between Summit Home Care, community, clients, and facilities
- Evaluates the outcome of care while providing support to ensure the services being rendered are meeting the client’s need and updates plan of care as needed
- Maintains correspondance with healthcare team and other staff of changes in the client’s condition or needs.
- Provides support to CHHAs in the home and conducts ongoing competency evaluations during each visit
