Salary Range:    $28.00 - $30.00 Hourly

Description

Hospice LPN - $3,000K Sign-on Bonus!!!!

Provide skilled professional nursing care to patient/family/caregiver in a home and nursing facility setting as directed by the VP Hospice or the RN case manager and in compliance with all applicable laws and regulations and with the VNA’s policies and procedures.  Provide timely documentation of patient services based on plan of care.  Identify other physical, psychological, social, and spiritual needs for assigned patients. Work closely with other members of the interdisciplinary team to ensure all patient and family care needs are being met.  Adhere to company policies and procedures and regulatory compliance requirements and directives. Travel as necessary.

What your responsibilities will be:

  • Ensure quality and safe delivery of home hospice services to patients/families/caregivers.
  • Implement nursing plan of care as directed by the interdisciplinary team.
  • Provide professional nursing care by utilizing all elements of nursing process as directed by the plan of care.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.
  • Responsible for checking and ordering medications for patients.
  • Assess and provide patient/family/caregiver education and information pertinent to diagnosis and self-plan of care.
  • Participates in coordination of Hospice services, appropriately communicating and reporting the need for Hospice interdisciplinary team members (e, g., PT, OT,MSW,ST, Dietitian) to the RN case manager.
  • Order and use equipment and supplies effectively and efficiently.
  • Provide appropriate pain/symptom management.  Evaluate patient’s response to treatments/medications.
  • Complete and maintain all patient records at patient’s residence and submit accurate and relevant clinical notes regarding patient’s condition and care given.
  • Observe patient for changes in condition and report changes and patient/family’s/caregiver’s needs to case manager or appropriate supervisory personnel.  Record pain/symptom management changes/outcomes as appropriate.
  • Assist patients with activities of daily living (e.g., reposition or move patients, bathing and feeding). Facilitate patient’s efforts toward self-sufficiency and optional comfort care.
  • Teach patient/family/caregiver self-care techniques as appropriate, working in conjunction with other members of the interdisciplinary team.
  • Provide and maintain a safe environment for the patient.
  • Fulfil productivity requirements on an average daily census basis as determined by acuity level and patient care management.
  • Assist patient/family/caregiver and other team members in providing continuity of care.
  • Work in cooperation with family/caregiver/and interdisciplinary team members to meet the emotional needs of the patient/family/caregiver.
  • Participates in evening/ weekend on-call conducting on-call services clinically competent and responsive manner.
  • Participate in staff meetings, department meetings, briefings, in-services committees and other related activities as needed.
  • Maintain clinical records, statistics, reports and records for purposes of evaluating and reporting of agency activities as prescribed and in compliance with local, state, and federal laws.
  • Assure for compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
  • Maintain all appropriate state nursing licensing requirements.  Attend continuing education courses.
  • Regular attendance is to be maintained.
  • Adherence to a code of conduct conducive to the VNA policy is expected.
  • Meet or exceed delivery of company service standards in a consistent fashion.
  • Interact with all staff in a positive and motivational fashion supporting the company’s mission.
  • Conduct all business activities in a professional and ethical manner.

COMMUNICATION:

  • Completes, maintains, and submits accurate and relevant clinical notes regarding patient condition and care given at the time the assessment is done or care given. Records pain/symptom assessments and management, changes/outcomes to pain/symptom management as appropriate.
  • Communicates with the primary care physician and RN case manager regarding the patient's needs and reports changes in patient condition; obtains/receives physician orders as needed.
  • Communicates with the facility staff or community health related persons to coordinate the plan of care for each assigned patient.
  • Teaches the patient and family/caregiver care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician recognizing and using opportunities for health counseling with patient and family/caregiver. Works in concert with the hospice Interdisciplinary Group/Team members.
  • Provides and maintains a safe environment for the patient.
  • Assists the patient, family/caregiver and other team members in providing continuity of care. Works in cooperation with the patient and family/caregiver and the hospice Interdisciplinary Group/Team members to meet the emotional needs of the patient and family/caregiver.
  • Attends the Interdisciplinary Group/Team meetings regularly.

ADDITIONAL DUTIES:

  • Ensures that equipment, medication and other necessary items and services are available to the patient.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Assumes responsibility for personal growth and development, maintaining and improving professional knowledge and practice skills through attendance and participation in continuing education programs.
  • Fulfills the obligation of LPN assignments.
  • Actively participates in quality management program.

PHYSICAL DEMANDS:  The demands described here are representative of those that must be met by an individual employee to successfully perform the essential functions of this job.  Ability to lift, pull, push Manipulate equipment and patients/clients which requires strength, gross motor, and fine motor coordination. Ability to follow plan of care.  Ability to sit and/or stand for long periods of time.   Ability to operate an automobile, Ability to climb stairs and provide services in the patient’s/client’s home environment.

Qualifications


POSITION QUALIFICATIONS
1. Graduate of an accredited practical nurse or vocational nursing program.
2. Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
3. Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
