The Spiritual Care Coordinator- (SCC) provides spiritual support to terminally ill patients and their families. The SCC will make spiritual assessments and bereavement assessments of newly admitted patients within five calendar days of admission; considering mental, emotional, physical and spiritual stresses, and respond with appropriate counseling and creating and individualized care plan.
SCC’s are members of the hospice interdisciplinary team, which is a group of specialists and professionals that provides guidance and support. Accordingly, they assist the medical staff in developing an individualized plan of care for each patient. Individualized plans may include spiritual consultations and administration of sacraments in accordance with ecclesial background and patients’ religious beliefs. SCC’s must be present for all IDT meetings.
The SCC is expected to serve in various roles related to the function of the organization as well. SCC's may be asked to meet with patients and families prior to admission to explain the scope of hospice services, to provide support group facilitation in partnership with long term care facilities, and to provide religious services in those facilities as part of Summit’s continued outreach efforts and integrated partnership efforts.
Essential Job Functions and Duties
- SCCs are expected to submit timely documentation in the electronic medical record on patient interactions
- Act as a liaison between members of the clergy and hospice patients. The SCC does not replace clergy members but rather is there to offer further support and communicate the needs of the patient
- Provide bereavement services, including phone calls, visits and memorial services, to family members up to thirteen months after a patient’s death
- May be required to participate in evening/weekend on-call rotations, to provide on-call spiritual support services in a timely manner
Position Qualifications:
- Summit Hospice requires Spiritual Care Coordinators to have a master's degree in divinity, Theology or a related discipline, as well as documentation of completion of at least one unit of Clinical Pastoral Education. We also require endorsement or ordination from a recognized religious body.
- The SCC should demonstrate a familiarity with the hospice philosophy and be able to communicate it to patients, families, and other members of the community, including facility partners and non-hospice clinicians.
