RN Supervisor for Home Care Services

Summit Home Care is a family-owned and operated home care agency. We are currently looking for compassionate and reliable per diem RN Supervisors to join our team of dedicated professionals. The RN supervisor is responsible for the oversight of their patient from admission through discharge.

Responsibilities:

• Works with Summit Patient Coordinators to ensure accurate clinical documentation for case opening and ongoing status of patient.
• Opens cases and initiates the CHHA plan of care and competency supervision
• Conducts Bi-monthly client assessments in-home and virtually as needed
• Is the liaison between Summit Home Care, community, clients, and facilities
• Evaluates the outcome of care while providing support to ensure the services being rendered are meeting the client’s need and updates plan of care as needed
• Maintains correspondance with healthcare team and other staff of changes in the client’s condition or needs.
• Provides support to CHHAs in the home and conducts ongoing competency evaluations during each visit
  
  
  

Requirements:

• Current New Jersey RN license
• Willingness to travel (90% travel) required (Monmouth and Ocean County)
• Reliable transportation
• 2 years experience supervising preferred
• Previous Home Care experience preferred
• Knowledgeable with NJ DOH regulations and policies
• Up to date first aid and CPR certification
• Exceptional staff training and customer service skills
• Compassionate and friendly demeanor
