Join Our Team as an RN Supervisor for PDN Services

Summit Home Care is a reputable, family-owned, and operated home care agency dedicated to providing compassionate care to our clients. We are currently seeking experienced and compassionate nurses to join our team of dedicated professionals.

RN Supervisor Responsibilities:

• Collaborate with the Patient Coordinator to ensure accurate clinical documentation for case openings.
• Initiate and revise the plan of care, medical orders, Medication Administration Records (MAR), and Treatment Authorization Requests (TAR) as needed.
• Conduct monthly client visits both in-home and virtually as required.
• Provide in-home supervision every 60 days or as necessary.
• Participate in formal and informal problem-solving processes to address EMR computer education needs and achieve established outcomes for users.
• Act as a liaison between Summit and the community, clients, facilities, etc.
• Evaluate the outcome of care and provide support to ensure that services meet the client’s needs.
• Inform physicians and other staff of changes in the client’s needs.
  
  
  

RN Supervisor Requirements:

• Current RN license in the state.
• 2 years of supervisory experience.
• Previous experience in Home Care.
• Exceptional staff training and customer service skills.
