Responsibilities:
- ASSESSMENT: Receives patient’s information and makes appointment to determine spiritual needs and concerns, establishing goals with patient and family including frequency of visits.
- VISITS: Presents appropriate spiritual material for patent and family based on expressed needs and preferences. Prays with the patient and/or family as invited and provides emotional support as appropriate. Documents visits and progress towards meeting goals.
- BEREAVEMENT: Performs or participates in bereavement services, including planning for viewings and funerals when requested. Provides spiritual support for survivors as requested.
- TEAMWORK: Attends IDT meetings and shares specific spiritual needs for each patient in their care. Attends in-service as appropriate and provides training as requested. Provides spiritual support for staff as invited.
Requirements
- Minimum of a two-year degree in the pastoral or other applicable field
- Previous experience in pastoral field, preferably healthcare-related
- Hospice experience preferred
