Dementia Care Advisor - Program Coordinator
Location: Indianapolis, Indiana – this position is a hybrid position but needs to be located in Central Indiana to be onsite with partners as needed.

Job Summary:
We are seeking a Dementia Care Program Coordinator to work alongside our partners to support PathWays members and their informal caregivers affected by Alzheimer’s and other related dementias. This tailored, person-centered service offering will provide the education, resources, and support to those affected by dementia to minimize common challenges inherent to the condition, maximize health outcomes and independence of the member with dementia, and improve caregiver proficiency while reducing caregiver stress and burden.
The Dementia Care Program Coordinator will identify, research, validate, build, and execute impactful health programs and initiatives. The role will be responsible for supporting multiple efforts, including conducting comprehensive research, building relationships with members and caregivers, designing new programs, and supporting the launch and execution of new initiatives. We are seeking an individual who exhibits a high level of professionalism, a passion for innovation, and a business style that is scrappy, strategic, creative, team-oriented, hands-on, and collaborative.
What You'll Do:
- Serve as an advocate and resource for members and caregivers who are impacted by Alzheimer's and other related dementia.
- Work alongside the health plan leadership identify new programs and initiatives in alignment with partners’ strategic plans.
- Coordinate and communicate with providers to support members, advocate for smooth process, and advocate for best practices.
- Build relationships with community organizations focused on dementia advocacy and research.
- Organize a process around program analysis, structuring, development, and execution.
- Participate in ongoing program developing including concept creation, project planning, analysis, execution.
- Research to find best-in-class program examples and health-related initiatives and work to include in partners’ strategy.
- Work closely with the Summit leadership team to develop and deliver key insights and opportunities related to program development.
- Support Summit’s process for delivering value at every partner interaction.
- Support the Summit leadership in documenting processes to create repeatable, scalable programs and initiatives.
- Seek out and build strong networks within healthcare program development.
What You'll Need:
- A bachelor's degree in public health, social work, healthcare administration, or a related field is preferred, but not required.
- A minimum of 2 years experience in external client-facing programs, case management, or other healthcare related field; or any combination of education and experience, which would provide an equivalent background.
- Demonstrate a high level of professionalism, integrity, and adherence to confidentiality standards.
- A compassionate and empathetic approach to understand clients' needs and provide appropriate support during their healthcare journey.
- Ability to create programs and initiatives to advance the goals of the partner strategy
- Detail-oriented and capable of managing multiple tasks efficiently while ensuring accuracy and meeting deadlines.
- Comfortable using computer systems, databases, and software applications for data entry and reporting purposes.
