What are the responsibilities and job description for the Jail and Community Case Manager position at Summit Psychological Associates, Inc.?
Summit Psychological Associates (SPA) has implemented a new and exciting program to assist men and women to successfully reintegrate into the community after a brief incarceration. SPA is hiring 1 additional full time case manager to work with inmates in the Summit County Jail and follow them upon release into the community. The goal for these critical case managers is to provide an inmate with a needs assessment and to help each inmate receive mental health treatment and substance use disorder treatment after their release.
Duties for Jail and Outpatient Case Managers:
Complete a thorough Needs Assessment for inmates related to their needs while incarcerated and upon release to the community.
Complete an Action Plan to direct case management services in the community.
Consult with clinical treatment staff in Summit County Jail, court officials and inmate support systems.
Meet regularly with inmates while incarcerated and develop a service plan for post incarceration and return to the community.
Assist with applying for available community benefits, including Medicaid and make appointments for outpatient services. Work with inmates who are homeless to secure housing services and emergency housing if necessary after release.
After release, assist with attendance at scheduled appointments related to treatment plan. Identify barriers to attending services and work to reduce those barriers.
Provide initial transportation to appointments if needed and work with client to secure ongoing transportation.
Communicate with agencies providing services to released inmates and ensure that services are effective.
Provide assistance and support in crisis situations, connect clients to appropriate crisis services.
Requirements/Preferences:
Associates or Bachelor's Degree in counseling, social work or psychology, preferred
Knowledge of community resources, preferred
Willingness to work with case management team and supervisors.
Ability to obtain NPI number and an Ohio Medicaid number
Detail oriented and self-motivated
Staff must show proof of car insurance and drivers license
