Position Overview :

We are seeking an organized and empathetic IHS (In-Home Support) Intake Coordinator to join our office team in Grand Junction, CO. This role offers a unique blend of independence and the opportunity to make a meaningful impact on the lives of community members. The Intake Coordinator serves as the first point of contact for clients seeking in-home care services and plays a vital role in ensuring smooth transitions for new clients and their families.

Key Responsibilities :

Handle incoming inquiries related to in-home support services, providing accurate and compassionate guidance.
Assess client needs and coordinate initial intake processes, including scheduling assessments and completing necessary documentation.
Serve as a liaison between clients, families, and internal care teams to ensure clear communication and a seamless onboarding experience.
Maintain up-to-date and organized client records in accordance with organizational policies and regulatory requirements.
Collaborate with team members to identify solutions that meet client needs and enhance overall service delivery.
Support the mission of SummitWest Care by fostering an inclusive and welcoming atmosphere for all clients and staff.

Qualifications :

Strong interpersonal and communication skills, with the ability to interact empathetically and professionally with diverse individuals

Experience in an administrative, healthcare, or customer service role is preferred
