Job Summary:
We are seeking a dedicated Interventionist to join our team. The Interventionist will work closely with families of children ages birth to three years of age who have developmental or related disabilities. The Interventionist will be responsible for developing an Individualized Family Service Plan (IFSP)/Family Service Plan (FSP) for each assigned child, planning and implementing developmental activities with family members; providing families with support, training, and instruction regarding identified areas of intervention; documenting all aspects of services rendered and activities performed; promoting positive community relations and developing inter-agency relationships.
Duties:
- Provide a minimum of four hours of family training a month, unless otherwise outlined in the IFSP/FSP
- Document all aspects of activities performed, including family training and service coordination activities; review of medical/ therapy reports/ progress notes; evaluation of progress towards established outcomes and needs and contacts with other agencies.
- Ensure that the child is assessed at least twice annually, or more often if needed, using an approved curriculum based assessment tool and that this activity is documented.
- Ensure that the DDSN Service Tracking System/ CDSS and the BRIDGES system are current and accurately reflect the services being received.
- Coordinate the transition to and from services (Early Head Start, Head Start, day care, public school, service coordination, etc.).
- Assess Early Childhood Outcomes Entry/ Exit ratings as required for children 0-3; complete the Child Outcomes Summary Form and input this information on the TECS database
- Coordinate and chair multi-disciplinary IFSP/FSP meetings (if the Early Interventionist is the primary service coordinator); perform service coordination services as requested by family members
Qualifications:
- Four-year degree in early childhood, special education, or related human services field from an accredited college or university; AND
- One year of experience working in the field of early childhood education, OR one year of experience working with infants and toddlers, OR one year of experience working with children age birth to five years with disabilities or special needs
- Ability to relate to individuals and families of diverse socio-economic backgrounds and races
- Ability to educate family members regarding the importance of early intervention
- Ability to provide instruction, training, support to families and evaluate their ability to perform required training
