Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Dietitian to join our team supporting Riverhead, Copiague, Brentwood, Wyandanch, and Amityville sites
The Dietitian plans and delivers nutritional information to assist in the promotion of health and control of disease.
Essential Responsibilities and Job Duties:
- Provides verbal, written, and/or demonstrations on various Medical Nutrition Therapies or general healthy eating based on patient's preferred education method considering health background, nutritional, cultural and socioeconomic history
- Evaluates medical records to obtain necessary medical information to determine client nutritional status, needs, and develop nutrition care plans
- Consults with physicians, nurse practitioners on appropriate nutritional intervention to improve client nutritional status
- Evaluates effects of nutritional intervention on individual client nutritional status to determine need for further intervention or revision of nutrition care plan
- Provides nutritional counseling and education for clients to assist them in making desired changes in eating habits.
- Provides consultation into the development of program policies and procedures
- Provides drug-nutrient interrelationship information to assure that clients receive maximum benefits from drugs and diet
- Attends staff meetings and trainings at both site and local levels, provides direction and executes health education programs as needed
- Maintains accurate records to meet regulatory agency guidelines and assists clients in making desired changes in eating habits
- Completes initial and follow up assessment encounters within the appropriate time frame according to policy and procedure
- Conducts in service/presentations as requested to clients enhance understanding of the role of nutrition in health and promote the role of the dietitian
- Attends and participates in seminars and continuing education programs to expand knowledge of current trends in nutrition and in issues/ concerns of community nutrition and public health
- Attends all mandatory in-service programs including fire safety, infection control, clinical HIV/AIDS manifestations, transmission, prevention psychosocial issues, confidentiality and client rights
- Maintains confidentiality of all pertinent client care information to ensure residents are protected
Minimum Education Requirement:
Bachelor’s Degree
Preferred Education Requirement:
Master’s Degree
Minimum Licensure/Certification/ Registration Requirement:
- RD/ RDN and CDN
- Must have a New York State License
- CPR/ BLS Certification
Salary Range: $53,835.60 to $60,587.80 / annually
