• Job Overview

Sun River Health is seeking a Licensed Master Social Worker (LMSW) for a full-time role (37.5 hours per week) in our Bronx locations. The LMSW will provide critical mental health and case management services to patients, including those affected by chronic illness, aging, maternity, and other social challenges. This role offers a $3,500 sign-on bonus (terms and conditions apply).

• Key Responsibilities
• Counseling and Crisis Intervention :
• Deliver supportive counseling, crisis intervention, and emergency evaluations for patients and their families.
• Intake and Assessment :
• Conduct mental health assessments and mental status exams for new patients, coordinating with primary care or psychiatric providers.
• Case Management :
• Manage a caseload, providing ongoing case management support and referrals as needed.
• Therapeutic Services :
• Facilitate individual, group, and family counseling sessions, supporting integrated care through collaborative group sessions with other providers.
• Resource Coordination :
• Connect patients to appropriate community resources and maintain thorough case documentation.
• Compliance and Quality Assurance :
• Ensure clinical services align with program grants, funding sources, and clinical standards.
• Patient-Centered Care :
• Develop and follow through on treatment plans in collaboration with patients and their families, promoting holistic care based on individual needs.
• Administrative Duties :
• Accurately document all clinical interactions, secure necessary service authorizations, and maintain up-to-date patient records.
• Qualifications
• Education :
• Master’s degree in Social Work (MSW) required.
• Licensure :
• Active New York State LMSW license required.
• Certifications :
• CPR / BLS certification (American Red Cross or American Heart Association, in-person only).
• Experience :
• Previous experience in behavioral health, crisis intervention, or case management preferred.
• Compensation
• Hourly Rate :
• 40.10 - $43.68 per hour, based on experience and licensure.
