Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Outreach Worker to join our team based out of our New Paltz site with travel as needed to specialty offices or other services provided in areas such as but not limited to Orange, Ulster, Dutchess counties.
An Outreach Worker will provide outreach services for clients and help to identify individuals and/or members of their families who are in need of preventative and primary health care services. S/He is responsible for providing education and prevention services to clients. For transporting patients you are assigned a company vehicle.
Outreach schedule
Monday 7:30am-6pm
Tuesday 9-5pm
Wednesday 7:30am-5pm
Thursday 7:30am-5pm
Fridays 7:30am-12pm
Essential Duties and Responsibilities:
- Assists in obtaining essential support services such as Medicaid, food stamps, transportation, translation services and day care.
- Serves as an advocate for the patients, and acts as a liaison between the patients and community agencies.
- Attends in-service education and training programs.
- Provides basic health information and education to patients.
- Arranges referrals and provides transportation as needed
- Completes daily activity logs, referral forms and other reports, as necessary.
- Provides all facility support for the Hudson Community and Health Center.
Minimum Education Requirement:
High School Graduate diploma/GED
Minimum Related Work Experience:
0-1 year of community outreach experience
Ability to communicate in English and Spanish (orally and written) required

Pay: $24.00 - $26.00 per hour
