Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Retention and Adherence Navigator to support our Poughkeepsie site.
SUMMARY OF POSITION:
Assists patients in navigating the health care system, links patients to medical care, provides education and support to promote engagement and retention in care. Utilizes motivational interviewing offering patient centered care promote adherence, viral load suppression, and reduce transmission. Assists patients in reducing barriers to care and improve health outcomes. Shares lived experience strategies with patients to address issues of chronic diseases. Educates patients about health education, harm reduction and safer sex practices.
ESSENTIAL FUNCTIONS:
- Orients new patients on RAP, health center services and staff.
- Links patients to primary care.
- Escorts patients to medical visits or other health and human service appointments
- Assists patients in navigating and accessing referral sources such as mental health and substance use services.
- Assists patients with accessing benefits/entitlements
- Provides support, education, and adherence assistance through individual or group activities
- Conducts reminder calls and follow-up for missed appointments.
- Conducts home visits as needed.
- Conducts targeted outreach to re-engage patients lost to care.
- Acts as a role model, mentor, and motivator for patients regarding adherence and retention.
- Facilitates patient education groups.
- Participates in ongoing training supervision, and professional development opportunities on for skill development.
- Participates in case conference, quality improvement projects, and collaboratives to share consumer perspective.
EDUCATION/EXPERIENCE:
High School Diploma/GED required
1-2 years of related work experience
Language:
Ability to communicate in English, verbal and written. Spanish optional.

Salary: $16.75 - $19.25 per hour
