Job Description
As a Home Health Registered Nurse at Sunbelt Staffing, you will deliver professional nursing care to patients in their homes, focusing on their comfort, safety, and well-being. You will conduct patient assessments, develop and implement care plans, and collaborate with a multidisciplinary team to ensure high-quality care.
Key Responsibilities
- Conduct comprehensive patient assessments to identify their needs and develop individualized care plans.
- Provide direct patient care, including wound care, medication administration, and disease management.
- Educate patients and their families on health management and disease prevention.
- Coordinate with physicians, physical therapists, and other healthcare professionals to ensure a cohesive approach to patient care.
- Utilize OASIS (Outcome and Assessment Information Set) for patient assessments and documentation.
- Use HCHB (Homecare Homebase) software for efficient care coordination and record-keeping.
