Job Title : Case Manager - Utilization Review
Location : Boise, Idaho
Job Description : We are seeking an experienced Case Manager specializing in Utilization Review to join our team for a 13-week contract assignment. This position involves providing discharge planning for adult patients in an acute inpatient hospital setting. The ideal candidate will have strong communication and delegation skills, be able to collaborate effectively with interdisciplinary teams, and prioritize patient-centered care.
Key Responsibilities :
Perform discharge planning for adult patients on all inpatient floors and ER.
Review patient cases to develop effective discharge plans, especially for high-risk patients, which includes conducting in-depth interviews.
Collaborate with providers, therapists, and case management assistants to coordinate care.
Utilize EPIC EMR for patient documentation and care coordination.
Work closely with case management assistants for administrative support, such as handling phone calls, faxing, and placing referrals as needed.
Maintain a community-oriented approach and foster a supportive team environment.
Required Qualifications :
Minimum of 2 years of experience as a Case Manager in an acute inpatient hospital setting.
BLS certification (must have or be willing to obtain prior to start).
Experience with EPIC EMR is preferred but not required.
CCM or ACM certification is preferred but not required.
Shift Details :
8-hour shifts from 7 : 30 a.m. to 4 : 00 p.m. (30-minute break).
May be required to work 10-hour shifts as discussed with the manager.
May cover 12-hour shifts (8 : 00 a.m. to 8 : 00 p.m.) if working in the Emergency Department.
Weekend shifts as needed (8-hour shifts from 7 : 30 a.m. to 4 : 00 p.m.).
May be required to work holidays if needed.
Contract Length : 13 weeks
