POSITION TITLE: PHARMACIST

Job Summary:
A pharmacist at a Federally Qualified Health Center (FQHC) ensures safe and effective medication use while promoting health equity for underserved populations. They collaborate with interdisciplinary teams to optimize treatment plans, provide patient education, and support chronic disease management. This role also involves ensuring compliance with regulatory standards, managing inventory, and contributing to community health initiatives.

Duties/Responsibilities:
- Accurately dispense prescriptions and counsel patients on medication use, side effects, storage, and chronic disease management.
- Collaborate with healthcare teams to optimize treatment plans and conduct medication therapy management (MTM).
- Address barriers to care, such as affordability and language, to improve medication adherence.
- Manage inventory levels, ensure the safe storage of medications, and monitor for adverse drug reactions and medication errors.
- Promote community wellness through health education programs and outreach initiatives.
- Ensure compliance with federal and state regulations and maintain proper documentation for HRSA and other funding agencies.
- Adhere to policies for drug procurement, storage, dispensing, and utilization to foster safe and cost-effective therapy.
- Participate in quality assurance (QA) and quality control (QC) activities to uphold high standards of care.
- Contribute to the planning and modernization of equipment and processes to enhance pharmacy operations.
- Optimize pharmacy medication expenditures and promote rational, safe drug therapy practices.
- Pursue continuing education and training to stay current with pharmaceutical care advancements.
- Perform additional tasks as needed to support the centers.

Education and Experience:
- Bachelor's Degree in Science is required.
- Doctor of Pharmacy (PharmD) degree is preferred.
- Valid Pharmacist Florida State License.
- Experience in ambulatory care or community health settings preferred.
- Knowledge of federal and state regulations related to FQHC operations and medication assistance programs.
- BLS is required within three months of hire.
Physical Requirements:
- Ability to stand for long durations while filling prescriptions and consulting with patients.
- Must be able to lift up to 25 pounds at times.
- Manual dexterity to handle small objects, such as pills, and operate dispensing equipment accurately.
- Ability to understand and/or interpret complicated program instructions and Florida laws.
- Must be able to travel to various center locations as required.
