Job Summary
The Registered Nurse provides high-quality clinical services and coordinates patient care plans with healthcare professionals in home and community settings. They ensure continuity of care by exchanging information with physicians, agency personnel, patients, families, case managers, and other community resources.
Key Responsibilities
- Delivers exceptional clinical services within scope of practice and infection control standards, adhering to plan of care/service plan and coordinating with care teams from admission through discharge.
- Conducts clinical nursing assessments as required by federal and/or state program requirements and payer sources.
- Evaluates patient eligibility and medical necessity for services defined by payers and agency policies.
- Develops and revises individualized plans of care and service plans according to federal and/or state program requirements and collaborative efforts with other community providers.
- Ensures plan of care frequency and duration meets patient needs, initiating revisions as needed with physician approval.
Desired Experience
- A minimum of one year experience as an RN is preferred.
Licensure Requirements
- Must hold current RN licensure in the state of practice.
- Requires a valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
- Current CPR certification is mandatory.
