Job Summary:
The Registered Nurse in Home and Community-Based Services delivers and oversees nursing and personal care services to patients in their homes as prescribed by the physician or authorized by the state/program, while adhering to applicable laws, regulations, and agency policies. The Registered Nurse coordinates the plan of care with other healthcare professionals involved in care, ensuring continuity of patient care through planning and exchanging information with physicians, agency personnel, patients, families, case managers, and community resources.
Key Responsibilities:
- Provides high-quality clinical services within the scope of practice, adhering to infection control standards, and in accordance with the plan of care/service plan, coordinating with other members of the patient/client's care team from admit through discharge.
- Conducts clinical nursing assessments in accordance with federal and/or state program requirements and as required by payer.
- Evaluates the patient/client's eligibility and medical necessity for services as defined by payer source and agency policy.
- Develops and revises individualized plans of care and/or service plans according to federal and/or state program requirements, collaborating with other community providers.
- Ensures the plan of care frequency and duration meets the patient's needs, initiating plan of care revisions as needed with physician approval.
Desirable Experience:
- A minimum of one year experience as an RN is preferred.
Required Licenses:
- Current RN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification is required.
