Job Overview

We are seeking a skilled and compassionate Registered Nurse to join our team as a PRN (per-diem) caregiver. As a member of our team, you will provide direct care to patients in their homes, working collaboratively with our interdisciplinary team to deliver holistic care and support services.

Key Responsibilities

1. Deliver high-quality, patient-centered care to individuals in need of home health services.

2. Develop and implement individualized care plans in consultation with patients, families, and healthcare professionals.
\
3. Maintain accurate and thorough documentation of patient care, including progress notes and medication administration records.
\
4. Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
\
5. Participate in ongoing education and training to stay current on industry developments and best practices.
\
\
1. Reports clinical findings, including changes in patient condition, to clinical manager/supervisor and physician timely.
\
2. Demonstrates flexibility with schedule to meet changing needs of assigned patients.
\
3. Administers medications per physician order and in accordance with federal/state/local regulations.
\
4. Transcribes physician orders accurately and timely.
\
5. Participates in patient case conferences with all disciplines of care team.
\
6. Maintains compliance with documentation requirements and electronic visit verification process.
\
7. Attends staff meetings as scheduled by agency and participates in performance improvement process.
\
8. Completes all orientation and annual training timely as required by organization and federal/state/local regulations.
\
9. Assists with orientation/training of agency staff as needed and performs competency evaluations/assessments for staff as assigned.
\
\
1. A minimum of one year experience as an RN preferred
\
2. Current RN licensure in state of practice required
\
3. Current CPR certification required
\
4. Valid driver's license and reliable transportation
