\Job Overview
\We are seeking a skilled and compassionate Registered Nurse to join our team as a PRN (per-diem) caregiver. As a member of our team, you will provide direct care to patients in their homes, working collaboratively with our interdisciplinary team to deliver holistic care and support services.
\Key Responsibilities

- Deliver high-quality, patient-centered care to individuals in need of home health services. \
- Develop and implement individualized care plans in consultation with patients, families, and healthcare professionals. \
- Maintain accurate and thorough documentation of patient care, including progress notes and medication administration records. \
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination. \
- Participate in ongoing education and training to stay current on industry developments and best practices. \
- Reports clinical findings, including changes in patient condition, to clinical manager/supervisor and physician timely. \
- Demonstrates flexibility with schedule to meet changing needs of assigned patients. \
- Administers medications per physician order and in accordance with federal/state/local regulations. \
- Transcribes physician orders accurately and timely. \
- Participates in patient case conferences with all disciplines of care team. \
- Maintains compliance with documentation requirements and electronic visit verification process. \
- Attends staff meetings as scheduled by agency and participates in performance improvement process. \
- Completes all orientation and annual training timely as required by organization and federal/state/local regulations. \
- Assists with orientation/training of agency staff as needed and performs competency evaluations/assessments for staff as assigned. \
- A minimum of one year experience as an RN preferred \
- Current RN licensure in state of practice required \
- Current CPR certification required \
- Valid driver's license and reliable transportation \
