Registered Nurse - Home and Community Based Services
We are seeking a compassionate and skilled Registered Nurse to provide high-quality clinical services to patients in their homes as part of our Home and Community Based Services team.
About the Role
The successful candidate will be responsible for providing and directing provisions of nursing and personal care to patients in their homes, as prescribed by physicians or authorized by state/program regulations. They will coordinate plans of care with other healthcare professionals involved in patient care and ensure continuity of care through planning and exchanging information with physicians, agency personnel, patients, families, case managers, and community resources.
Key Responsibilities
- Provide high-quality clinical services within scope of practice and infection control standards, in accordance with plan of care/service plan and coordination with other members of patient/client's care team from admission through discharge.
- Complete clinical nursing assessments in accordance with federal and/or state program requirements and as required by payer.
- Ensure patient/client's eligibility and medical necessity for services as defined by payer source and agency policy.
- Develop and revise individualized plans of care and/or service plans according to federal and/or state program requirements with other community providers.
- Ensure plan of care frequency and duration meets needs of patient and initiates plan of care revisions as needed with physician approval.
- Provide oversight and supervision of plan of care/service plan as required by agency policy, federal, and/or state program and as required by payer, including services provided by licensed practical nurse and paraprofessional, for assigned caseload.
- Communicate assessment findings and perform care coordination to physician, case manager, and/or other appropriate members of patient/client's care team.
Requirements
- Current RN licensure in state of practice.
- Current drivers license, vehicle insurance, and access to dependable vehicle or public transportation.
- Current CPR certification required.
Preferred Experience
A minimum of one year experience as an RN is preferred.
