Key Responsibilities
- Assess patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions
- Implement care plans in collaboration with multidisciplinary teams, ensuring patient-centered care and coordination
- Provide skilled nursing services, including medication management, wound care, and mobility support
- Document patient interactions and progress accurately and efficiently
- Maintain open communication with patients, families, and healthcare professionals to ensure seamless care transitions
Requirements
- Bachelor's degree in Nursing or equivalent experience
- Current RN licensure in the state of GA
- 1 year of experience as an RN, preferably in home health or hospice settings
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
