Job Summary
The Registered Nurse will work with patients and families to develop and implement personalized care plans, coordinate services, and ensure seamless transitions between healthcare settings. The ideal candidate will have a passion for home-based care, excellent assessment and documentation skills, and the ability to work effectively in a fast-paced environment.
Key Responsibilities
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing effective care plans
- Collaborate with patients, families, and other healthcare professionals to coordinate care and services, ensuring a comprehensive and coordinated approach
- Provide direct patient care, including administering medications, performing treatments, and providing education and support
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements
- Communicate effectively with patients, families, and colleagues, using clear and respectful language
Requirements
- Bachelor's degree in nursing required; master's degree preferred
- Current RN licensure in state of practice; current CPR certification also required
- A minimum of one year of experience as an RN, preferably in home health or hospice care
Preferred Qualifications
- Experience working with diverse patient populations, including children, adults, and older adults
- Strong knowledge of medical terminology, anatomy, and physiology
- Excellent time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities
