Job Summary
">We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Skilled Nursing Home Visit nurse. As a member of our team, you will provide skilled nursing care to patients in their homes, working closely with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
Key Responsibilities:
">- Provide skilled nursing care to patients in their homes, including administering medications, wound care, and other treatments as prescribed by physicians.
- Conduct initial and ongoing assessments of patients' physical and emotional needs, developing and implementing care plans that meet those needs.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate care and ensure continuity of services.
- Document patient progress and outcomes, maintaining accurate and complete records of all interactions with patients and healthcare providers.
Requirements
">- Curent RN licensure in state of practice.
- CPR certification.
- One year of experience in home health care or related field, preferred.
