Job Description:
The Licensed Practical Nurse (LPN) in Home Health is responsible for providing technical skilled care to patients in their homes, ensuring they receive the highest level of care and support.
- Provides services within the scope of practice as defined by state laws governing nursing practice, in accordance with the plan of care, and in coordination with other healthcare team members. May not alter the plan of care.
- Instructs patients and their families on self-care techniques and maintenance of a healthy environment for the patient.
- Observes, records, and reports patient responses to treatment and changes in condition daily to supervising nurses and/or physicians.
Requirements:
- Current LPN licensure in state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
